The VA in San Jose California is a general medicine clinic which includes a variety of specialty support services. Lean Integrated has partnered with Rona Consulting Group (now part of Moss Adams) to develop Lean management processes in preparation for the move to the new community based outpatient clinic (CBOC).
While the average patient wait time at this clinic is currently seventeen minutes, the providers are striving to cut that time in half. Integration of support services into the patient appointment will ensure optimal care for our veterans. In preparation for the move to the new CBOC, now under construction, the team is seeking to streamline the patient experience. While patients may schedule appointments simply for annual exams, the providers may discover previously undisclosed issues. Many of these issues can be time sensitive. Immediate response by a mental health professional, social worker, or medical specialist, can mean the difference between unnecessary suffering and a significantly improved quality of life.
The goal of the San Jose clinic is to optimize the integration of these services through standardized “provider to provider” patient handoffs. These “warm handoffs” will ensure patients receive the care they require at the moment they require it.
